Perhaps one of the expert railfans can explain how such a derailment could occur, but it seems rather odd to me. It looks like the first four cars of the train proceeded straight through the switch normally, but then the fifth car went off down a different branch. I'm guessing the most likely cause would be if the switch actually changed positions while the train was traveling through; but then, how could that happen?

It looks like a switch failure: A train was maneuvering through the yard, and one of the cars decided to head off in a different track.

Maybe the switch wasn't properly disengaged and the first four cars had shaken it loose? Seems energetically improbable, but I take the Orange Line way too often to think about what else could have gone wrong.

The first three cars went over the switch to the left, the fourth car "split the switch", one end went to the left, the other went over the switch to the right, the 5th and 6th cars began to follow car 4 to the right.
