T worker facing discipline for Green Line door panel disappearance

A Green Line inspector could be in hot water for an incident on the Green Line this morning in which a door panel on a trolley suddenly fell off the train, a spokesman says:

A Green Line inspector is facing disciplinary action for failing to ensure that the trolley door was secured in the closed position before the train proceeded to Park St. Because of his failure to follow established procedures, a segment of the door dislodged after making contact with a part of the subway infrastructure.

The incident contributed to a general mess on the Green Line this morning.

I thought trains were not supposed to be able to move if all doors are not fully closed.

The train was disabled and being pushed by another one (four cars coupled together). The first two cars were on door interlock-bypass so it could be moved.

When trains are disabled and have to be put on bypass the door in question should of been isolated which looks like didn't happen in this case. Safety issue ! Yes it can happen because I'm sure the inspector was in a hurry trying to move the train and all but you have to go by what your taught and that is to go step by step so you don't miss something.

Back in the early days of the Boeing LRVs, when they had the 600 parts per pair plug doors, when a pair of doors had to be isolated, they would use a rubber strap that fit over lugs on the inside of the doors - the idea being to prevent the doors from inadvertently opening.

If I am correct, there is no such provision for using such a strap on the inside of the folding doors on either the Type 7s or Type 8s.

I also find it curious that a piece of "subway infrastructure" would be so close to the cars edge that an extended door could easily strike it.

There are MANY places where columns, signals, signs, etc. are mere inches from the sides of cars. In some places even the tunnel walls are that close.
