I haven't been on this U-turn yet. Can someone explain how this happened? Are trucks banned from that thing or did someone just fall asleep at the wheel?

Daniel said the crash does not signify a design flaw: The road is not supposed to carry long tractor-trailers and the driver tried to make too tight of a turn.

No signs declaring vehicle limits
Coming back from the airport to Brighton the other day, I used the A-B exit and saw the new ramp. Nothing comes to mind from the signage that suggests that it's a limited access road/ramp. If there's supposed to be some sort of limit to the road, I can't remember seeing it Sunday...and that suggests to me that it's not marked well enough if it didn't leave any impression at all.
Also, when the Cambridge/Boston side of the tolls at the A-B exit gets backed up, it would frequently go up the ramp quite a way...but previously if you still wanted to go to Allston/Brighton, you could use the left lane to bypass that mess and use the other half of the toll booth. The ramp now injects itself into that left lane and makes the 4-lane toll booth start from just the right line traffic...that's going to keep A/B drivers stuck on the ramp longer waiting for the slow and busy C/B lights to let people off of the Pike and into Cambridge/Storrow.

I've been watching them build this new ramp for over a year. (My commute takes me out of the city westbound on the Pike.) Being a relative newcomer I was naive enough to think the Pike was using my toll money to build something that might actually prove useful to me once in a while... until I learned recently that it's restricted to taxis, buses and commercial vehicles that are short enough to actually make it around the bend without doing a Fung Wah.
Anonymous presumably saw several large signs with stuff like "U-Turn to Boston", "Fast Lane tags only", "No trucks over 3 axles". But there are also two much smaller signs - blink and you'll miss them - with the restriction information. No doubt the cops are gearing up to issue tickets to unsuspecting out-of-towners who get in their rental car at Logan and follow the signage to Boston west on I-90.

Kind of like the way the "No trucks" sign is missing from the Boylston St. onramp to Storrow Dr. westbound. You've gotta wonder about the intelligence level of the people that maintain these roads.

I saw the new ramp on Monday afternoon and noticed the "Fast Lane tags only" signs, but not the other restrictions.
