When you first set up your account, do not pick one of the offered starter pokemon, but walk/run away from them. They will follow for a while, and then give up. At that point, a pikachu will present himself and you can pick him as your starter.

My starter Pokemon have left and I have been walking "alone" for about 20 minutes, when is the pikachu supposed to come out? Still on initial screen where it says "catch a Pokemon"
